1. Why a single blood panel is useless without a baseline
Most enhanced lifters pull labs once mid-cycle, panic over a number, and either do nothing useful or overreact. A single panel is a snapshot. Without a pre-cycle baseline taken in the same lab, at the same time of day, under similar conditions, you cannot tell what is a real change versus normal individual variation.
The minimum useful cadence is:
- Baseline: 2–4 weeks before the cycle, fasted, well-hydrated, no acute illness.
- Mid-cycle: 4–6 weeks in, or sooner if symptoms appear.
- Post-cycle / cruise: 4–6 weeks after cessation or after reaching a stable cruise dose.
Track the trend, not the frame. A lipid shift that looks alarming in isolation may be predictable and manageable when you see the slope.
2. The core panel
Ask for a comprehensive metabolic panel plus hormones, not a basic testosterone check:

3. HDL/LDL: what the drop means and how fast is too fast
Testosterone and especially oral androgens lower HDL and can raise LDL within weeks. The absolute numbers matter, but the rate of change from baseline matters just as much.
Practical targets for most healthy enhanced athletes:
- HDL: stay above 40 mg/dL and within roughly 30% of your baseline.
- LDL: watch the absolute rise, not just the ratio. A jump >50 mg/dL from baseline deserves attention.
- Triglycerides: ideally under 150 mg/dL.
Levers to pull if lipids move the wrong way:
- Increase EPA+DHA intake to 2–4 g per day from fish oil or algae oil.
- Raise soluble fiber to 25–40 g/day (oats, beans, psyllium, legumes).
- Reduce long-chain saturated fat and trans fats.
- Add regular low-intensity cardio (30–45 minutes, 3–5x/week).
- Limit or remove oral androgens if HDL collapses or LDL spikes sharply.
4. AST/ALT: oral vs injectable differences
Injectable testosterone typically has minimal impact on AST and ALT. Oral 17-alpha-alkylated androgens can raise both enzymes because they pass through the liver before entering circulation.
Rules of thumb:
- A 2–3x rise from baseline is common with orals and often resolves after cessation.
- A >3x rise, or any rise accompanied by itching, jaundice, dark urine, or right-upper-quadrant pain, warrants stopping orals and seeing a clinician.
- Do not rely on liver “support” supplements to justify continuing a hepatotoxic stack.
Support that has actual evidence: N-acetylcysteine 600–1200 mg/day, and avoiding the combination of multiple oral compounds. Milk thistle has mixed data; TUDCA may help in specific cholestatic patterns but is not a license to keep running orals.
5. Hematocrit: why androgens raise it and what to do
Androgens stimulate erythropoietin, increasing red blood cell mass. The result is thicker blood, higher blood pressure, and a higher cardiovascular load.
General guidance:
- Aim to keep hematocrit under 54%.
- If it is climbing toward that threshold, first address hydration, cardio load, and sleep.
- Donating blood or therapeutic phlebotomy can lower hematocrit, but it is a last resort, not a first-line fix. It also depletes iron and ferritin, which can cause fatigue and other problems. If phlebotomy is used, monitor ferritin.

6. E2, SHBG and free testosterone: interpreting the hormonal picture
Total testosterone tells only part of the story. SHBG binds testosterone; when SHBG drops — which orals and high-dose androgens do aggressively — free testosterone rises even if total T looks stable.
Estradiol (E2) follows aromatization. The goal is not a specific number; it is the symptom-to-hormone ratio. Low SHBG plus moderate E2 can feel high-E2 because more E2 is unbound. High SHBG plus high total T can still leave free T and E2 effects muted.
Symptom patterns to watch:
- Low E2: dry joints, low libido, flat mood, poor sleep.
- High E2: water retention, gynecomastia sensitivity, mood swings, erectile issues.
- Low SHBG: inflated free hormones, harder to interpret standard ranges.
Adjust protocol based on symptoms and repeated labs, not one isolated E2 value. Aromatase inhibitors are a last resort; start with dose timing, injection frequency, and body composition changes.
7. When to pause, adjust support, or see a doctor
Pause or seek medical review if you see any of the following:
- Hematocrit ≥54% with headaches, dizziness, or elevated blood pressure.
- AST/ALT >3x baseline, or any signs of jaundice.
- eGFR dropping trend or creatinine rising sharply.
- HDL <30 mg/dL or LDL >190 mg/dL, especially with family cardiac history.
- Chest pain, shortness of breath, unilateral leg swelling, or severe mood disturbance.
Adjust support before escalating to prescription intervention:
- Hydration, cardio, and fiber first for lipids and hematocrit.
- NAC and oral cessation for liver stress.
- Injection frequency and dose review for E2/SHBG symptoms.
